Roles & Responsibilities

• Provide direct nursing care to residents with compassion and professionalism
• Administer medications and treatments accurately and on time
• Monitor patient conditions and communicate changes to the care team
• Document care provided in accordance with facility standards
• Collaborate with nurses, physicians, and interdisciplinary staff
• Educate patients and families on health management and care plans

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania
• Graduation from an accredited practical nursing program
• Current BLS/CPR certification (American Heart Association preferred)
• Previous LPN experience in long-term care or skilled nursing a plus
• Strong clinical and critical-thinking skills

Knowledge & Skills

• Medication administration and dosage calculation
• Understanding of resident care plans and activities of daily living
• Proficient in electronic health records (EHR) documentation
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassionate, patient-centered approach to care
