Roles & Responsibilities

• Provide compassionate nursing care to residents and patients under the supervision of registered nurses and physicians
• Administer medications and treatments accurately as prescribed
• Monitor and document patient vital signs, reporting any changes to the care team
• Assist with daily living activities to promote patient comfort and independence
• Maintain accurate and timely documentation of care provided
• Collaborate with an interdisciplinary team to ensure high-quality, personalized care

Qualifications

• Current LPN license in Pennsylvania or compact state
• BLS/CPR certification required
• Previous long-term care or acute care experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and teamwork abilities

Knowledge & Skills

• Knowledge of nursing principles and practices
• Medication administration and dosage calculations
• Patient assessment and vital sign monitoring
• Electronic health record (EHR) documentation
• Infection control and safety procedures
