Licensed Practical Nurse (LPN) - Per Diem
Location: Lancaster, Pennsylvania
Start Date: 08-22-2026
Contract Type: Per-Diem
Shift Type: Flexible 8-hour shifts; days and start times may vary based on facility needs
Hours per Week: 40
Candidate Pay Rate: $34 per hour

Roles & Responsibilities
- Provide compassionate, patient-centered care to residents and patients in a long-term care setting
- Administer medications and treatments accurately and safely per physician orders
- Monitor vital signs and assess patient conditions, reporting changes to the care team
- Document patient care thoroughly and maintain accurate electronic health records
- Assist with activities of daily living and support rehabilitation goals to promote independence
- Collaborate with registered nurses, physicians, and interprofessional teams to deliver high-quality care
Qualifications
- Active Licensed Practical Nurse (LPN) license in Pennsylvania
- Current BLS/CPR certification from the American Heart Association or equivalent
- Previous experience in long-term care, skilled nursing, or rehabilitation is a plus
- Strong clinical assessment and critical thinking abilities
- Ability to adapt quickly to new environments and work as part of a dynamic team
Knowledge & Skills
- Solid knowledge of medication administration, wound care, and patient assessments
- Comfortable using electronic health records (EHR) systems for documentation
- Excellent verbal and written communication skills
- Ability to work independently while also thriving in a collaborative team environment
- Compassionate and respectful approach when caring for residents and their families
