Roles & Responsibilities

• Provide compassionate, patient-centered care to residents and patients in a long-term care setting
• Administer medications and treatments accurately and safely per physician orders
• Monitor vital signs and assess patient conditions, reporting changes to the care team
• Document patient care thoroughly and maintain accurate electronic health records
• Assist with activities of daily living and support rehabilitation goals to promote independence
• Collaborate with registered nurses, physicians, and interprofessional teams to deliver high-quality care

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS/CPR certification from the American Heart Association or equivalent
• Previous experience in long-term care, skilled nursing, or rehabilitation is a plus
• Strong clinical assessment and critical thinking abilities
• Ability to adapt quickly to new environments and work as part of a dynamic team

Knowledge & Skills

• Solid knowledge of medication administration, wound care, and patient assessments
• Comfortable using electronic health records (EHR) systems for documentation
• Excellent verbal and written communication skills
• Ability to work independently while also thriving in a collaborative team environment
• Compassionate and respectful approach when caring for residents and their families
