Roles & Responsibilities

• Provide direct patient care, including medication administration and treatment procedures
• Monitor and document patient vitals, reporting any changes to the healthcare team
• Assist patients with daily living activities and personal hygiene with compassion
• Collaborate with nurses and physicians to develop and implement care plans
• Educate patients and families on health conditions and aftercare
• Maintain accurate and timely patient records in compliance with facility policies

Qualifications

• Current and unencumbered LPN license in New Mexico or hold a compact license
• BLS certification (American Heart Association) required
• COVID-19 vaccination or documented medical/religious exemption as required by facility
• Valid government-issued photo ID and ability to wear a photo badge at all times
• Prior LPN experience in a long-term care or skilled nursing setting is a plus, but new grads are welcome

Knowledge & Skills

• Strong clinical skills including medication administration and wound care
• Excellent observation and assessment abilities
• Effective communication skills with patients, families, and team members
• Proficiency with electronic health records (EHR) systems
• Ability to work 12-hour night shifts and adapt to a fast-paced environment
• Compassionate, patient-centered approach to care
