Roles & Responsibilities

• Provide high-quality bedside care to patients, focused on safety, dignity and comfort
• Administer medications and treatments per physician orders and facility protocols
• Monitor patient status and document changes accurately in the medical record
• Collaborate with RNs, therapists and interdisciplinary team members to coordinate care
• Perform wound care, IV site checks and other skilled nursing tasks as appropriate
• Support patient and family education, answering questions with empathy and clarity
• Assist with admissions, transfers and discharge planning to ensure smooth transitions

Qualifications

• Current Licensed Practical Nurse (LPN) credential — active and in good standing
• Basic Life Support (BLS/CPR) certification required or willing to obtain
• Current immunizations and ability to pass required background and drug screening
• Comfortable working overnight shifts and managing clinical priorities independently
• Previous experience in long-term care, rehab or acute care preferred but motivated new grads are welcome to apply

Education

• Diploma or certificate as a Licensed Practical Nurse (LPN); completion of an accredited LPN program

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Proficient charting and documentation practices (electronic medical record experience a plus)
• Excellent communication and teamwork abilities
• Ability to prioritize tasks in a fast-paced environment
• Compassionate patient interaction and family communication skills
