Roles & Responsibilities

• Evaluate patients' physical therapy needs and develop individualized treatment plans
• Implement evidence-based therapeutic exercises and modalities to improve mobility and function
• Monitor patient progress and adjust treatment approaches to achieve optimal outcomes
• Collaborate with nurses, physicians, and the rehab team to deliver coordinated, compassionate care
• Educate patients and families on home exercise programs, fall prevention, and safe mobility techniques
• Document patient evaluations, progress notes, and discharge summaries in compliance with facility standards

Qualifications

• Active Physical Therapist license in Pennsylvania (or willingness to obtain before start)
• Graduation from an accredited physical therapy program
• Current BLS/CPR certification
• Previous experience in skilled nursing or rehabilitation settings preferred
• Strong clinical and documentation skills with attention to detail
• A compassionate, patient-centered approach to care

Knowledge & Skills

• Solid understanding of evidence-based physical therapy practices and modalities
• Skill in conducting comprehensive patient assessments and developing care plans
• Proficient with electronic medical records (EMR) and documentation systems
• Excellent communication and interpersonal skills for team collaboration and patient education
• Strong time management and organizational abilities in a fast-paced rehab environment
• Knowledge of Medicare and insurance documentation requirements
