Shift Type: Per diem, 8-hour shifts; exact start/end times and days vary based on facility needs

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide safe, compassionate bedside care to residents based on care plans
Administer medications and treatments accurately and document appropriately
Monitor resident status and promptly report changes to the care team
Collaborate with nurses, CNAs, therapists, and physicians to deliver coordinated care
Perform thorough charting and maintain accurate clinical records
Assist with admission, transfer, and discharge processes as needed
Support infection control practices and maintain a safe care environment
Advocate for resident needs and contribute to a person-centered care culture

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification or willingness to obtain
Demonstrated ability to work independently and as part of a team
Comfortable with charting, medication administration, and ADL support
Ability to pass required background checks and health screenings

Education

Graduate of an accredited practical nursing program or equivalent training

Knowledge & Skills

Solid clinical nursing fundamentals and medication administration knowledge
Strong observation and assessment skills for acute and chronic conditions
Clear, timely documentation and communication skills
Ability to prioritize tasks in a fast-paced skilled nursing environment
Familiarity with infection control and resident safety practices
