Roles & Responsibilities

Provide compassionate, patient-centered care in accordance with facility policies
Administer medications and treatments accurately and document accordingly
Monitor patient condition, report changes, and escalate concerns promptly
Collaborate with nurses, CNAs, and interdisciplinary team members to ensure continuity of care
Assist with activities of daily living while promoting patient dignity and independence
Maintain accurate and timely clinical documentation and participate in care planning

Qualifications

Valid LPN/LVN license in Pennsylvania (required)
Completion of an accredited practical nursing program
Current Basic Life Support (BLS) certification or willingness to obtain
Strong interpersonal skills and the ability to work well within a care team
Reliable, punctual, and able to meet the physical demands of the role

Education

Completion of accredited Practical Nursing (LPN) program; High school diploma or GED required

Knowledge & Skills

Medication administration and safe documentation practices
Basic assessment skills and recognizing changes in patient condition
Familiarity with electronic health records and charting
Infection control, wound care basics, and safe patient handling
Good communication skills with patients, families, and clinical teams
