Roles & Responsibilities

• Provide direct, compassionate nursing care to residents in a skilled nursing setting
• Administer medications and treatments accurately and safely
• Monitor and document patient status, changes, and responses to interventions
• Collaborate with the interdisciplinary care team to develop and implement care plans
• Support admissions, transfers, and discharges with clear, timely documentation
• Promote a safe, clean environment and follow infection control best practices
• Educate residents and families about care plans, medications, and follow-up needs

Qualifications

• Current, active LPN license (Licensed Practical Nurse) — required
• Travel LPN (T-LPN) experience is a plus and welcomed
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Ability to pass background check, drug screen, and required facility onboarding
• Comfort working night shift hours and collaborating with a cross-functional team
• Strong communication skills and a resident-focused approach to care

Education

• Graduate of an accredited Practical Nursing program; LPN credential required

Knowledge & Skills

• Knowledge of medication administration, resident assessment, and documentation
• Familiarity with long-term care/skilled nursing workflows and regulations
• Proficient in charting and electronic health record documentation
• Excellent prioritization and time-management skills during busy shifts
• Team player mentality with good clinical judgment and problem-solving
