Roles & Responsibilities

• Provide compassionate outpatient physical therapy services tailored to each patient's goals
• Perform thorough evaluations and develop individualized treatment plans
• Deliver hands-on manual therapy and therapeutic exercise interventions
• Coordinate care with Home Health and Inpatient teams for seamless transitions
• Document patient progress accurately and in a timely manner
• Educate patients and families on home exercise programs, safety, and prevention
• Contribute to a positive clinic culture by living the core values of faith, family, fairness and fun

Qualifications

• Active Minnesota Physical Therapist license or eligibility to obtain MN licensure
• Graduation from an accredited physical therapy program (DPT preferred)
• Comfortable working primarily in outpatient settings with occasional home health and inpatient duties
• Strong documentation and communication skills; able to work independently and as part of a team
• Willingness to complete a 4-week onboarding period prior to starting patient caseload

Education

• Doctor of Physical Therapy (DPT) preferred; graduation from an accredited physical therapy program required

Knowledge & Skills

• Solid knowledge of anatomy, kinesiology, and therapeutic exercise principles
• Hands-on manual therapy techniques and therapeutic modalities
• Experience with outpatient evaluation and treatment planning
• Ability to adapt care across outpatient, home health, and inpatient settings
• Proficient communication and patient education skills
• Familiarity with electronic medical records and accurate clinical documentation
