Roles & Responsibilities

• Provide skilled assessment and treatment for outpatient speech, language, and communication disorders
• Develop individualized, evidence-based treatment plans that drive measurable progress
• Document patient evaluations, treatment plans, and progress notes in the medical record accurately and timely
• Collaborate with interdisciplinary team members to coordinate patient-centered care
• Educate patients and families on therapy goals, home programs, and strategies to support carryover
• Maintain professional standards and comply with facility policies, state regulations, and ethical guidelines

Qualifications

• Active Speech-Language Pathologist license in the state of Nebraska
• BLS certification from the American Heart Association (BLS for Healthcare Providers)
• Current professional credentials in good standing and ability to meet facility credentialing requirements
• Strong clinical reasoning, organization, and documentation skills
• Comfort working independently in an outpatient setting and collaborating with a care team

Knowledge & Skills

• Skilled in standardized and functional speech-language assessments
• Competent in creating individualized, evidence-based treatment plans
• Knowledgeable about dysphagia screening/management and relevant precautions
• Experience with patient and family education and home program development
• Familiarity with common outpatient documentation practices and electronic medical records
• Effective communication and interdisciplinary collaboration skills
