Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Provide compassionate care coordination and support to patients in need
Conduct thorough assessments and develop individualized care plans
Collaborate with interdisciplinary teams to ensure seamless patient care
Facilitate involuntary evaluations and coordinate appropriate psychiatric holds when necessary
Advocate for patients and connect them with vital community resources

Qualifications

Active LCSW license in California
Current BLS certification
5150 Certification in California required
Prior experience working with Dignity Health facilities
Strong clinical assessment and care coordination skills

Knowledge & Skills

Expertise in psychiatric evaluation and involuntary holds
Knowledge of California mental health regulations
Excellent communication and documentation abilities
Ability to work collaboratively within a multidisciplinary team
Compassionate approach to patient advocacy and support
