Location: Carmichael, California

Start Date: 06-23-2026

End Date: 09-22-2026

Contract Type: Contract

Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, up to every other weekend

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Lead meaningful goals-of-care discussions that truly matter to patients and families
Assess patient needs and develop individualized care plans with compassion
Educate patients and families on disease processes, advance directives, and POLSTs
Facilitate family meetings and provide emotional support during end-of-life journeys
Collaborate with physicians and the interdisciplinary team during daily rounds
Manage a rewarding caseload of 5-6 new patients and 3-4 follow-ups each day

Qualifications

Active California RN license required
2-5 years of recent palliative care or hospice experience
BLS certification required
ELNEC certification preferred
Strong acute care, clinical thinking, and communication skills
ICU or oncology experience is a plus

Knowledge & Skills

Cerner EMR proficiency
Palliative care screening and consultation expertise
Excellent organizational and flexibility skills
Compassionate approach to serving patients, families, and team members
Strong clinical judgment in acute care settings
