Roles & Responsibilities

• Provide skilled nursing care, assessment, planning and implementation for hospice patients in their homes
• Admit patients to hospice services and perform thorough patient/family instruction and education
• Coordinate and integrate interdisciplinary services to ensure continuity and quality of care
• Perform and document OASIS-compliant assessments and maintain accurate patient records
• Oversee Home Health Aides and CNAs, ensuring care aligns with the plan and quality standards
• Manage symptom control and provide compassionate end-of-life support to patients and families
• Participate in team scheduling and on-call rotations, triaging telephone concerns and completing home visits as needed
• Contribute to quality improvement activities and attend in-service training to keep skills sharp

Qualifications

• Current Registered Nurse (RN) license in the State of Montana (required)
• BSN preferred (candidates with ADN considered)
• Minimum 2 years recent acute care, home care, or hospice experience preferred
• Clinical skills such as venipuncture, catheterization, wound care and/or parenteral therapy preferred
• Valid driver's license, appropriate auto insurance, and reliable transportation for home visits
• Comfort participating in an on-call rotation and responding to patient needs promptly

Education

• Active RN license (Montana)
• BSN preferred

Knowledge & Skills

• Strong patient assessment and clinical judgment in home/hospice settings
• Experience with OASIS documentation and regulatory documentation standards
• Care coordination and case management across interdisciplinary teams
• Symptom management and palliative/hospice nursing principles
• Comfort providing hands-on skills: wound care, venipuncture, catheter care, parenteral therapy
• Excellent communication, teaching and family support skills for challenging conversations
• Ability to manage time, documentation, and travel for scheduled home visits
