Roles & Responsibilities

• Provide compassionate, patient-centered care to residents and patients
• Administer medications and treatments as prescribed
• Monitor patient vitals and document changes in condition
• Collaborate with physicians and multidisciplinary teams to create individualized care plans
• Educate patients and families on health management and post-discharge care
• Maintain a safe, clean, and supportive environment

Qualifications

• Active Registered Nurse (RN) license in Pennsylvania
• Current BLS/CPR certification
• Graduate from an accredited nursing program
• Previous experience in long-term care, skilled nursing, or acute care preferred
• Ability to work as-needed shifts, including weekends and holidays as needed

Knowledge & Skills

• Strong patient assessment and critical thinking skills
• Proficient in medication administration and IV therapy
• Excellent communication and documentation abilities
• Knowledge of electronic health records (EHR) systems
• Ability to work autonomously and as part of a team
• Empathy, patience, and a positive attitude
