Roles & Responsibilities

• Coordinate safe, timely discharges and next-level care transitions for inpatients
• Conduct thorough needs assessments and order DME when appropriate
• Develop and manage individualized plans of care focused on optimal outcomes
• Document patient care and case management activities accurately in Epic
• Collaborate closely with physicians, nurses, social services, and other stakeholders
• Work across hospital areas, including Colby and Pacific campuses, to meet patient needs
• Support regulatory compliance related to CMS guidelines and utilization review
• Cover every-other-weekend shifts and adapt to floating between units as needed

Qualifications

• Active RN license required (pending licenses accepted) — MSW may be considered for certain case management roles
• BLS certification required
• Minimum 1 year acute hospital experience with discharge planning or care coordination
• Experience with case management tasks: needs assessment, DME ordering, plan-of-care development
• Comfort working with high patient ratios (experience with 1:25–30 days is noted)
• Willingness to float across units and work both Colby and Pacific campuses (campuses are <2 miles apart)
• Ability to work every other weekend as part of the rotation

Education

• Registered Nurse (RN) required or Master of Social Work (MSW) for eligible case management roles

Knowledge & Skills

• Proficient charting in Epic
• Strong knowledge of acute care workflows and discharge planning
• Familiarity with CMS regulations and utilization review processes
• Experience with Med-Surg, LLOS, ED, ICU environments preferred
• Excellent care coordination and interdisciplinary communication skills
• Ability to assess needs, arrange DME, and create effective plans of care
