Registered Nurse - Care Coordinator
Location: Santa Cruz, California
Start Date: 06-01-2026
End Date: 08-31-2026
Contract Type: Contract
Shift Type: Days, Monday-Sunday, 7:00 AM - 7:00 PM, 12-hour shifts, 36 hours per week guaranteed, every other weekend, first to float
Hours per Week: 36
Candidate Pay Rate: $82 per hour

Roles & Responsibilities
- Coordinate patient care seamlessly across the team for optimal outcomes
- Advocate for patients as their dedicated care coordinator in a dynamic acute setting
- Collaborate with multidisciplinary staff to ensure smooth transitions and exceptional care
- Utilize Cerner charting system to document and track progress effectively
Qualifications
- Active RN license required
- BLS and ACLS certifications mandatory
- 2 years of experience as Care Coordinator RN
Education
- BSN not required
Knowledge & Skills
- Proficiency in case management within acute care environments
- Strong coordination and communication skills
- Familiarity with Cerner electronic health records
