Roles & Responsibilities

• Implement plan of care under PT supervision — deliver skilled interventions to help residents progress
• Provide hands-on therapy and assist residents with mobility, strengthening, balance, and functional tasks
• Document timely and accurately in Nethealth and PCC to reflect resident progress and compliance
• Collaborate with the rehab team and onsite Director of Rehab to coordinate resident care
• Meet department productivity expectations (90%) while maintaining patient-centered care
• Follow Continuum Rehabilitation Group (CRG) policies & procedures and facility dress code (black scrubs)
• Be present and clocked-in at the facility to receive guaranteed hours and ensure continuity of care

Qualifications

• Licensed Physical Therapist Assistant (PTA) in the state of Missouri (active and in good standing)
• 1–3 years of clinical experience (job requests 1+ years experience)
• Comfortable and experienced with electronic documentation — Nethealth and PCC experience a plus
• Ability to work in a skilled nursing facility (SNF) rehab environment and collaborate with interdisciplinary teams
• Reliable, professional presence onsite with adherence to facility policies and productivity expectations
• Current BLS/CPR preferred (if required by facility), immunizations and background checks as per facility onboarding

Education

• Graduate of an accredited Physical Therapist Assistant program (Associate degree or equivalent)
• Current PTA licensure in Missouri (state licensure required)

Knowledge & Skills

• Strong documentation skills and familiarity with Nethealth and PCC
• Clinical knowledge of SNF rehabilitation practices and skilled nursing workflows
• Good communication and teamwork to coordinate with therapists, nursing, and leadership
• Time management and productivity focus to meet departmental goals
• Patient-centered approach to deliver safe, effective, and compassionate care
• Ability to adapt to facility protocols and workflow while maintaining professional standards
