Physical Therapy Assistant - Home Health
Location: Lexington, Kentucky
Start Date: 07-27-2026
End Date: 10-26-2026
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekends and holidays
Hours per Week: 40
Candidate Pay Rate: $35 per hour

Roles & Responsibilities
- Provide exceptional physical therapy services in patients' homes
- Conduct comprehensive evaluations and develop treatment plans
- Perform start of care, recertifications, evaluations, and daily visits
- Help patients recover from injuries and improve mobility
- Work autonomously with support from a dedicated clinical team 24/7
- Utilize Homecare Homebase for efficient charting and documentation
- Make a meaningful difference in the comfort of patients' homes
Qualifications
- Active Physical Therapy Assistant license in Kentucky
- BLS certification (AHA required)
- Minimum 1 year of experience as a PTA
- Home health experience preferred but not required
- Willing to accept first-time travelers (no new grads please)
- Reliable transportation and valid driver's license
Education
- Associate's degree in Physical Therapy Assisting from an accredited program
Knowledge & Skills
- Proficient in Homecare Homebase (HCHB)
- Strong clinical assessment and problem-solving skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Compassionate and patient-centered approach to care
- Knowledge of home health regulations and OASIS documentation
