Roles & Responsibilities

• Perform thorough patient evaluations to assess physical abilities and needs
• Develop individualized treatment plans focused on restoring function and independence
• Provide evidence-based physical therapy interventions to improve strength, mobility, and balance
• Educate patients and families on therapeutic exercises, safe movement, and home modification strategies
• Collaborate with nurses, occupational therapists, physicians, and the interdisciplinary team to coordinate care
• Monitor patient progress, adjust plans as needed, and document outcomes to optimize results
• Maintain timely, accurate documentation in accordance with facility policies and regulatory requirements

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license (ability to obtain California licensure required if not currently licensed)
• Experience in a skilled nursing facility or post-acute setting preferred
• Strong knowledge of therapeutic modalities and adaptive equipment
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work independently while contributing positively to a team-focused environment
• Compassionate, patient-centered approach to care

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
