Location: Louisville, Kentucky

End Date: 09-21-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide comprehensive physical therapy services in home health settings
Conduct start of care assessments and evaluations for new patients
Perform recertifications and resume of care visits
Deliver daily therapy visits to patients with CHF, orthopedic conditions, and pneumonia
Collaborate with clinical managers and educators for patient care coordination

Qualifications

Current Physical Therapist license required
BLS certification required
Home health experience preferred
No new grads preferred

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and evaluation skills
Strong communication and documentation abilities
Ability to work independently in field settings
Comfort with technology for documentation
