Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Deliver personalized physical therapy care directly in patients' homes
Assess and treat patients with orthopedic conditions, strokes, joint replacements, and spinal injuries
Complete OASIS documentation to ensure compliance with government requirements
Collaborate with interdisciplinary teams including OT and Speech Therapy
Develop individualized treatment plans that promote independence and recovery
Build meaningful relationships while helping patients regain mobility and confidence

Qualifications

Active Physical Therapist license in California
BLS certification required
Minimum 1 year of experience working with PT/OT/Speech populations
Experience with OASIS documentation required
Home health setting experience preferred but not required
Cerner EMR experience preferred

Knowledge & Skills

Strong assessment and clinical reasoning skills
Excellent communication and patient education abilities
Comfortable working independently in home-based settings
Proficiency with OASIS documentation and Cerner EMR
Compassionate approach to patient-centered care
