Physical Therapist - Home Health
Location: Rancho Cordova, CA
Start Date: 08-04-2026
End Date: 11-03-2026
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:30 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Provide physical therapy treatments in patients' homes, including assessments, evaluations, and plan of care
- Supervise up to 2 PTAs or COTAs
- Complete OASIS documentation and case management
- Collaborate with RN manager and rehab team
- Travel between patient homes within service territory
Qualifications
- CA Physical Therapist License required
- BLS certification (AHA) required
- 2 years experience in healthcare setting, home health preferred
- Current driver's license and auto insurance
- Experience with Homecare Home Base (HCHB) preferred
Knowledge & Skills
- Knowledge of OASIS documentation
- Strong organizational and communication skills
- Ability to work independently in home environments
- Time management and patient scheduling skills
