Roles & Responsibilities

• Perform thorough home health evaluations and skilled physical therapy visits in patients' homes
• Develop individualized home exercise programs and teach patients and families how to carry them out safely
• Provide patient and family education to support functional independence and successful home transitions
• Document care accurately and efficiently in EPIC and complete OASIS submissions as required
• Collaborate with nurses, case managers, and other clinicians to coordinate care and optimize outcomes
• Manage a typical caseload of up to 6 visits per day while maintaining high-quality, patient-centered care
• Travel between client homes safely and professionally, minimizing drive time while maximizing patient contact

Qualifications

• Current Physical Therapist license for the state of Oregon (pending licenses accepted)
• Minimum 1 year of skilled home health experience required; first-time travel therapists not accepted
• BCLS certification required
• Proof of primary series COVID vaccination plus one booster as required by facility
• Reliable personal transportation and ability to travel within the assigned radius
• Experience with OASIS and EPIC charting required

Knowledge & Skills

• Home Health clinical skills and home environment assessment
• OASIS documentation and EPIC charting
• Creating effective Home Exercise Programs
• Patient and family education techniques
• Orthopedics: total hip and total knee replacement, post-op shoulder and cervical procedures
• Neurologic rehab: post-stroke rehabilitation, spinal cord injury, traumatic brain injury, Parkinson's disease, multiple sclerosis
• Body mechanics and safe transfer/mobility training
• Assessment skills across acute, outpatient, rehab, and SNF settings
