Physical Therapist Assistant - Home Health
Location: Santa Maria, CA
Start Date: 07-20-2026
End Date: 10-19-2026
Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with flexible start times, every 3rd weekend
Hours per Week: 40
Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Provide physical therapy services to patients in their homes under the supervision of a PT
- Complete OASIS assessments and documentation
- Collaborate with interdisciplinary team to ensure coordinated care
- Implement and adjust treatment plans based on patient progress
- Educate patients and families on exercise programs and home safety
Qualifications
- Current Physical Therapist Assistant license in California
- BLS certification required
- Minimum 2 years of experience in physical therapy
- Experience with OASIS documentation required
- Home health experience preferred
Knowledge & Skills
- Knowledge of OASIS documentation and government mandated forms
- Proficient in Cerner EMR
- Strong communication and interpersonal skills
- Ability to work independently in home health setting
