Roles & Responsibilities

• Develop and implement individualized physical therapy plans for residents
• Conduct therapeutic exercises and modalities to improve mobility and function
• Document patient progress using Net Health and iPads
• Collaborate with interdisciplinary team to coordinate care
• Maintain compliance with facility policies and regulatory standards
• Provide compassionate care in an assisted living & memory care setting

Qualifications

• Licensed Physical Therapist in West Virginia
• Minimum 0 years of experience (entry‑level acceptable)
• Strong communication and teamwork skills
• Ability to work 35 hours per week, Monday‑Friday
• Commitment to delivering high‑quality care

Knowledge & Skills

• Physical therapy techniques and modalities
• Rehabilitation for memory care populations
• Electronic health record documentation
• Interdisciplinary collaboration
• Patient safety and infection control
