Roles & Responsibilities

• Provide compassionate, skilled occupational therapy evaluations and develop individualized treatment plans
• Deliver evidence-based interventions to improve residents' independence with activities of daily living
• Collaborate closely with nursing, PT, SLP and interdisciplinary teams to coordinate care
• Document clinical progress and discharge planning clearly and timely
• Support coverage at the sister outpatient location for minimal treatments as needed
• Participate in team meetings and contribute to quality improvement initiatives

Qualifications

• Active or eligible Minnesota Occupational Therapy license (able to obtain MN licensure prior to start)
• Entry-level to experienced OTR candidates welcome — new grads encouraged to apply
• Comfortable floating between skilled nursing and outpatient settings when needed
• Strong communication skills and a team-oriented mindset
• Reliable transportation and flexibility to cover sister site shifts

Education

• Master’s or Doctorate in Occupational Therapy (or equivalent professional degree)

Knowledge & Skills

• Skilled in ADL training, therapeutic exercise, and adaptive equipment recommendations
• Familiarity with skilled nursing facility documentation and care pathways
• Excellent clinical reasoning and patient-centered care planning
• Ability to collaborate effectively within an interdisciplinary team
• Time management skills to balance caseload and coverage responsibilities
