Roles & Responsibilities

• Evaluate patients' functional status and develop individualized occupational therapy plans of care
• Complete thorough assessments and reassessments using objective tests and measurements
• Deliver home-based therapy that helps patients regain independence in self-care, leisure, and work activities
• Document care accurately and concisely, capturing treatments, communications, and coordination
• Communicate treatment observations and recommendations as an active member of the interdisciplinary team
• Provide patient and caregiver education to support safe home functioning and ongoing recovery
• Adapt quickly to changes in patient condition and respond to emergent needs with clinical judgment
• Respect and protect patient rights while conveying empathy and support during difficult moments

Qualifications

• Licensed by the State of Oregon as an Occupational Therapist (required)
• Graduate of an accredited occupational therapy curriculum and eligible for the National Registration Examination of the American Occupational Therapy Association
• Current certification in BLS (required)
• Valid driver’s license and proof of auto insurance (required) — must have own car for patient home visits
• Registration with the American Occupational Therapy Association preferred
• CRASE training required if travel assignment extends beyond six months (preferred)
• Evidence of continuing education in physical disability/rehabilitation, medicine and surgery procedures, mental health concepts, geriatric and long-term care (desired)

Education

• Graduate of an occupational therapy curriculum accredited by the Committee on Allied Health Education and Accreditation (required)
• Eligible for the National Registration Examination of the American Occupational Therapy Association (required)

Knowledge & Skills

• Patient assessment and objective measurement for home health and hospice settings
• Treatment planning and implementation focused on meaningful daily activities
• Clinical documentation and accurate communication across the care team
• Care coordination and interdisciplinary team collaboration
• Home visit best practices, safety assessment, and adaptive equipment recommendations
• Patient and caregiver education with a compassionate, person-centered approach
• Experience with geriatric care, long-term care needs, and mental health concepts relevant to OT
