Roles & Responsibilities

Provide compassionate direct patient care and support daily living needs
Administer medications and treatments safely and accurately
Monitor, record, and report patient vital signs and condition changes
Collaborate with nurses, therapists, and the care team for coordinated care
Educate patients and families about care plans and health maintenance
Maintain clear, accurate documentation in patient charts and EMR
Follow infection control, safety protocols, and facility policies

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification (required or obtained upon hire)
Previous LPN experience in long-term care, rehab, or similar clinical settings preferred
Strong communication skills and a team-oriented mindset
Ability to pass background check and drug screening as required by facility

Education

LPN diploma or certificate (required)

Knowledge & Skills

Medication administration and basic IV/therapy familiarity
Patient assessment and monitoring of vital signs
Wound care basics and pressure injury prevention
EMR/charting and accurate documentation practices
Infection control and safe patient handling
Time management, prioritization, and teamwork
