Roles & Responsibilities

• Deliver compassionate, resident-centered nursing care based on individualized care plans
• Administer medications and treatments safely and accurately
• Assess resident condition and report changes to the care team promptly
• Document clinical observations and interventions in the medical record
• Collaborate with nurses, therapists, and support staff to promote quality outcomes
• Support activities of daily living and encourage resident independence
• Follow infection control and safety protocols consistently
• Provide clear, respectful communication with residents and families

Qualifications

• Active LPN license in Pennsylvania required
• Graduation from an accredited LPN program
• Current BLS/CPR certification preferred or required upon hire
• Previous experience in skilled nursing or long-term care preferred
• Clean background check and ability to meet facility health requirements (immunizations as required)
• Strong communication skills and a team-oriented attitude

Education

• LPN diploma or certificate from an accredited program
• High school diploma or GED

Knowledge & Skills

• Patient assessment and basic clinical skills
• Medication administration and documentation
• Charting in electronic or paper medical records
• Infection control practices and safety procedures
• Time management and prioritization in a busy care setting
