Roles & Responsibilities

Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
Monitor and document resident condition and vital signs accurately
Administer medications and treatments as directed and follow care plans
Communicate effectively with residents, families, and the interdisciplinary team
Assist with activities of daily living and promote resident comfort and dignity
Report changes in patient status promptly to RN/supervisory staff and participate in care planning

Qualifications

Current Licensed Practical Nurse (LPN) license valid in Connecticut
CPR/BLS certification (or willingness to obtain upon hire)
Strong attention to detail and solid charting/documentation skills
Compassionate bedside manner and excellent communication skills
Ability to pass required background checks and meet facility onboarding requirements

Education

LPN diploma or completion of an accredited practical nursing program

Knowledge & Skills

Knowledge of basic nursing procedures and medication administration
Familiarity with long-term care or skilled nursing workflows preferred
Ability to follow care plans and communicate clinical observations clearly
Basic computer skills for electronic charting and documentation
Team-oriented mindset and ability to collaborate with interdisciplinary staff
