Roles & Responsibilities

• Provide compassionate, patient-centered care to residents in a skilled nursing and rehabilitation center
• Administer medications, treatments, and injections as prescribed, and monitor for side effects
• Perform routine nursing assessments and promptly report changes in patient condition to the RN or physician
• Collaborate with the care team to develop and update individualized care plans
• Document patient care accurately and thoroughly in the electronic medical record
• Educate patients and families on health conditions, medications, and post-discharge care to promote wellness

Qualifications

• Active and unencumbered LPN license in the state of Maine
• Current BLS/CPR certification
• Prior experience in long-term care, skilled nursing, or rehabilitation preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to stand, walk, and lift as required in a clinical setting
• Compliance with facility requirements, including COVID-19 vaccination or approved medical/religious exemption

• Medication administration and documentation
• Wound care and basic life support
• Patient assessment and vital sign monitoring
• Electronic health record (EHR) proficiency
• Compassionate bedside manner and active listening
