Roles & Responsibilities

• Provide direct, compassionate nursing care to residents in a long-term care setting
• Administer medications and treatments accurately and on time
• Monitor resident health, vital signs, and changes in condition, reporting concerns promptly
• Collaborate with RNs, physicians, and care team members to develop and adjust care plans
• Document care provided and maintain accurate resident records
• Support residents with daily living activities, promoting independence and dignity

Qualifications

• Active LPN license in the state of practice or ability to obtain one
• BLS/CPR certification required (American Heart Association or similar)
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Strong clinical assessment and critical thinking skills
• Ability to adapt quickly to new teams and environments
• Completion of COVID-19 vaccination or documented exemption required

Knowledge & Skills

• Medication administration and safety protocols
• Resident care planning and documentation using EMR/EHR systems
• Understanding of infection control practices
• Clear communication with residents, families, and healthcare teams
• Empathy, patience, and a positive bedside manner
