Roles & Responsibilities

• Provide compassionate, direct patient care to assigned residents and patients
• Administer medications and treatments according to facility protocols and physician orders
• Perform thorough assessments and monitor patient vitals, reporting changes promptly
• Document care accurately and timely in the patient record or EMR
• Collaborate with nurses, CNAs, and interdisciplinary team members to deliver coordinated care
• Support safe patient transfers, mobility assistance, and fall-prevention measures
• Educate patients and families about care plans, medications, and follow-up needs

Qualifications

• Current, active LPN (Licensed Practical Nurse) license for New Mexico (required)
• Basic Life Support (BLS/CPR) certification required or must obtain upon hire
• Current immunizations and TB screening per facility policy
• Previous LPN experience in long-term care, skilled nursing, or similar setting preferred
• Comfortable with medication administration, wound care basics, and charting
• Able to pass background check and meet facility onboarding requirements
• Great communication skills and a collaborative, patient-first mindset

Education

• Vocational nursing diploma, certificate, or equivalent; active LPN license required

Knowledge & Skills

• Strong clinical assessment skills and ability to recognize patient changes
• Medication administration and documentation accuracy
• Familiarity with infection control standards and safe nursing practices
• Competence with electronic medical records and charting
• Time management and ability to prioritize patient needs on busy shifts
• Team-oriented attitude with clear, compassionate communication
