Location: Missoula, Montana

Start Date: 05-27-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with on-call and weekend rotation

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled nursing and compassionate case management for patients receiving care in their homes
Coordinate seamless care transitions and advocate for patients and families throughout their healthcare journey
Assess, plan, and implement individualized care plans while monitoring patient progress and outcomes
Educate patients and families on managing symptoms and improving quality of life
Collaborate with an interdisciplinary team to ensure continuity of care and optimal clinical results
Maintain accurate documentation and support quality improvement initiatives

Qualifications

Current, active RN license in the State of Montana
Minimum 2 years of recent acute care, home care, or hospice experience
Valid driver's license, appropriate auto insurance, and reliable transportation
BSN preferred; HCHB experience strongly preferred
Strong clinical skills including venipuncture, catheterization, wound care, and parenteral therapy

Skilled Nursing and comprehensive patient assessment

Patient and family education and counseling
Interdisciplinary collaboration and communication
OASIS documentation and current reimbursement guidelines
