Roles & Responsibilities

• Provide compassionate direct care including feeding, hygiene, grooming, and mobility support
• Assist with ambulation, transfers, and positioning using gait belts and mechanical lifts to keep residents safe
• Support restorative and range-of-motion exercises to help residents stay as independent as possible
• Respond promptly to call lights and attend to resident needs with empathy and professionalism
• Document ADLs, intake/output, restorative care, and other assigned tasks accurately in the EMR prior to end of shift
• Observe and report changes in resident physical, emotional, or cognitive status to the Licensed Nurse
• Assist with prosthetics, colostomy care, and adaptive equipment while maintaining resident dignity and privacy
• Maintain a clean, safe, and organized environment — stock supplies, make beds, and assist with laundry
• Follow infection control protocols including PPE, hand hygiene, and transmission-based precautions
• Participate in onboarding and training of new staff and contribute to a positive unit culture
• Support dining room setup, social engagement, and community-based activities to promote quality of life
• Assist in emergency procedures and respond appropriately to codes and security alerts

Qualifications

• Active CNA certification on the Wisconsin Nurse Aide Registry (must be valid at time of submission)
• Active BLS (Basic Life Support) certification required at time of submission
• 2+ years of CNA experience
• Ability to lift 50+ lbs and support residents safely using proper body mechanics and equipment
• Computer literacy with experience documenting in electronic medical record (EMR) systems
• Willingness to work across all units including long-term care, rehab, dementia, and ventilator units (refusal to work specific units may lead to contract cancellation)
• Reliable, compassionate, and a strong team player with good communication skills

• Activities of Daily Living (ADL) assistance

• Ventilator care awareness and end-of-life/hospice support
• Dementia care strategies and behavior support

• Effective communication, teamwork, and resident-centered care planning
• Ability to respond quickly to resident needs and emergency situations
