Roles & Responsibilities

• Coordinate comprehensive care and discharge planning to keep patient transitions safe and timely
• Manage prior authorizations and continued stay reviews to support appropriate utilization
• Collaborate with interdisciplinary teams to develop and document individualized plans of care in Epic
• Perform needs assessments, order DME, and verify benefits eligibility to remove barriers to care
• Conduct utilization review, apply admission criteria and CMS/DRG/HEDIS guidance to optimize patient outcomes
• Float between units as needed, providing flexible coverage and maintaining high-quality case management

Qualifications

• Minimum 1 year of acute care case management or charge experience in a hospital setting
• Experience managing patient ratios up to 1:25
• Proficient with Epic charting system
• Must have a reliable vehicle for this assignment
• BLS preferred
• Case Management Certification preferred
• Ability to float between units and work every other weekend as scheduled
• Travel experience not required

• Care coordination and discharge planning
• Prior authorizations and benefits eligibility
• Continued stay reviews and utilization review
• Admission criteria, DRG, and CMS regulations
• HEDIS, NCQA, HIPAA, OSHA, and The Joint Commission standards
• Plan of care development and needs assessment/DME orders
• Familiarity with behavioral health, home health, hospice, LTAC/Rehab/Skilled Nursing work settings
• Strong communication, documentation, and interdisciplinary collaboration skills
