Roles & Responsibilities
- Provide occupational therapy services under the supervision of an Occupational Therapist to help residents regain independence
- Assist in developing and implementing individualized treatment plans focused on functional goals
- Conduct assessments and evaluations to identify patient needs, strengths, and realistic goals
- Administer therapeutic exercises and activities to improve motor skills, coordination, and daily living tasks
- Train patients and caregivers on use of adaptive equipment and assistive devices to maximize safety and independence
- Monitor and document patient progress clearly and promptly, adjusting plans as needed
- Collaborate with nursing and the interdisciplinary team to deliver comprehensive, person-centered care
Qualifications
- Certified Occupational Therapy Assistant (COTA) credential required
- Current state registration/licensure or eligibility to practice in Arizona
- Experience in a Skilled Nursing Facility (SNF) or geriatric/rehabilitation setting preferred
- Strong clinical documentation skills and familiarity with assessments and treatment planning
- Excellent interpersonal and teamwork abilities for interdisciplinary collaboration
- Up-to-date immunizations, background check, and ability to meet facility onboarding requirements
- BLS/CPR certification preferred
Education
- Associate degree from an accredited Occupational Therapy Assistant (OTA) program or equivalent COTA education
Knowledge & Skills
- Occupational Therapy principles and therapeutic interventions
- Patient assessment and evaluation techniques
- Treatment planning and goal-setting for functional outcomes
- Therapeutic exercises and activity-based interventions
- Use and training of adaptive equipment and assistive devices
- Clinical documentation and progress tracking
- Interdisciplinary collaboration within a Skilled Nursing Facility
