Roles & Responsibilities

• Collaborate with Physical Therapists to implement individualized treatment plans that help patients regain mobility and independence
• Guide residents through therapeutic exercises and activities, adjusting techniques to meet their unique needs
• Track and document patient progress, celebrating milestones and communicating updates to the care team
• Educate patients and families on exercises and techniques to continue progress at home
• Support a compassionate, positive environment in our skilled nursing facility

Qualifications

• Active Physical Therapist Assistant license in Massachusetts
• Great communication and teamwork skills
• Experience in skilled nursing or rehabilitation settings is a plus
• Ability to lift and assist patients as needed
• Compassionate, patient-focused approach

Knowledge & Skills

• Knowledge of therapeutic exercises and rehabilitation techniques
• Understanding of documentation standards and HIPAA
• Skilled in using electronic health records
• Strong interpersonal and active listening skills
• Ability to adapt to changing patient needs
