Physical Therapist Assistant - Home Health
Location: Rancho Cordova, California

Contract Type: Contract
Shift Type: Monday-Friday, 8-hour day shifts
Hours per Week: 40
Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients in a home health setting
- Provide skilled physical therapy interventions to help patients regain independence
- Manage 5-6 patient visits per day with a focus on quality care
- Collaborate with healthcare team to develop and adjust treatment plans
- Document patient progress accurately using Cerner charting system
Qualifications
- Active California Physical Therapist Assistant license
- Current BLS certification from AHA
- Minimum 2 years of experience as a PTA preferred
- Valid driver's license and reliable transportation for home visits
- Ability to provide 2 supervisor references (1 within 12 months, 1 within 3 years)
- Must not have been directly employed by a CommonSpirit facility within the past year
Knowledge & Skills
- Experience in home health or community-based therapy
- Proficiency with Cerner electronic medical record system
- Strong time management skills to handle a daily caseload of 5-6 patients
- Excellent communication and patient education abilities
- Adaptability to treat a variety of diagnoses and patient needs
