Roles & Responsibilities

• Provide direct patient care to residents and visitors
• Administer medications and treatments accurately
• Document patient assessments and care plans
• Collaborate with interdisciplinary team members to optimize outcomes
• Assist with patient education and discharge planning

Qualifications

• Licensed Practical Nurse (LPN) license in Pennsylvania
• Minimum of 0 years experience (entry-level welcome)
• Strong communication and teamwork skills
• Ability to work independently and adapt to changing priorities

Knowledge & Skills

• Patient assessment and vital sign monitoring
• Medication administration and IV therapy
• Wound care and dressing changes
• Charting and electronic health record documentation
• Patient education and discharge instructions
