Contract Type: Per-Diem

Shift Type: 8-hour shift; specific start/end times and days not provided by facility (schedule to be confirmed with recruiter)

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, resident-centered nursing care consistent with LPN scope
Administer medications and treatments safely and accurately
Monitor and document vital signs, changes in condition, and care provided
Communicate clearly with residents, families, and interdisciplinary team members
Assist with care planning and follow established care protocols
Maintain accurate medical records and complete required documentation in a timely manner
Support a positive, respectful environment that promotes resident dignity and independence

Qualifications

Current, active LPN license or eligible for LPN licensure in Pennsylvania
CPR/BLS certification required or must obtain upon hire
Demonstrated ability to deliver compassionate direct patient care
Willingness to complete background check and required health screenings
Strong communication, teamwork, and basic clinical documentation skills

Education

Graduate of an accredited practical nursing program
LPN diploma or certificate required

Knowledge & Skills

Medication administration and basic wound care
Resident assessment and monitoring of vital signs
Familiarity with long-term care workflows and best practices
Accurate charting and documentation skills
Good interpersonal skills and a patient-centered approach
