Contract Type: Travel Contract

Shift Type: 40 hours/week, flexible between 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM, weekends as needed

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide high-quality physical therapy services to patients in a home health setting
Develop and implement individualized treatment plans
Collaborate with healthcare teams to ensure comprehensive care

Qualifications

Current Physical Therapy license
BLS certification required
Experience in home health or similar setting preferred
Epic system experience preferred but not required

Education

Physical Therapy degree from an accredited program

Knowledge & Skills

Strong clinical skills in physical therapy
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
