Roles & Responsibilities

• Provide compassionate, patient-centered care to residents in a skilled nursing environment
• Administer medications and treatments accurately and document promptly
• Assess patient conditions, recognize changes, and communicate with the care team
• Collaborate with nurses, physicians, and therapists to implement individualized care plans
• Supervise and support certified nursing assistants in daily resident care
• Maintain a safe, clean, and comforting environment for patients and families
• Educate residents and families on care plans and health management

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS certification (American Heart Association preferred)
• Prior long-term care, skilled nursing, or geriatric experience is a plus
• Strong clinical judgment and ability to work independently
• Comfort with flexible, per diem scheduling
• Excellent communication and teamwork skills

Knowledge & Skills

• Medication administration, wound care, and vital sign monitoring
• Knowledge of electronic health records and charting systems
• Understanding of infection control and safety protocols
• Ability to prioritize and adapt in a fast-paced environment
• Compassionate communication with residents and families
• Team collaboration and delegation to support staff
