Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care environment
• Administer medications and treatments accurately and document per facility policy
• Monitor resident status, record vital signs, and report changes to the care team
• Collaborate with RNs, CNAs, therapists, and physicians to deliver coordinated care
• Assist with admissions, transfers, and discharge planning when needed
• Support rehabilitation goals and daily living activities to maximize resident dignity
• Maintain accurate clinical documentation in the resident chart and electronic records
• Follow infection control standards and facility policies to ensure a safe environment

Qualifications

• Active LPN license in New York (required)
• Completion of an accredited practical nursing program
• Current BLS/CPR certification (or willing to obtain prior to start)
• Ability to pass background and health screenings per facility requirements
• Prior experience in long-term care or skilled nursing preferred but new grads considered
• Strong communication, teamwork, and resident-focused bedside manner

Education

• Practical Nursing Certificate or Diploma from an accredited program; high school diploma or GED required

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and skin integrity awareness
• Proficient in taking and interpreting vital signs
• Familiarity with electronic health record (EHR) charting or willingness to learn
• Good clinical judgment and the ability to escalate concerns appropriately
• Excellent interpersonal skills for working with residents and families
