Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents in a skilled nursing facility
• Administer medications and treatments as ordered, while monitoring residents for changes in condition
• Collaborate with the care team to develop and update individualized care plans
• Document patient assessments, interventions, and outcomes accurately and promptly
• Communicate effectively with residents, families, and healthcare professionals
• Support a safe, respectful, and comfortable environment for residents

Qualifications

• Current Licensed Practical Nurse (LPN) license in Pennsylvania
• BLS certification required
• Previous experience in long-term care or skilled nursing preferred, but new grads are welcome to apply
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work per diem/as needed shifts

Knowledge & Skills

• Knowledge of LPN scope of practice and medication administration
• Understanding of resident care plans and documentation standards
• Ability to respond calmly and effectively in emergencies
• Compassionate and patient-centered approach
• Comfortable using electronic health records and basic medical equipment
