Roles & Responsibilities

• Provide compassionate, patient-centered care to residents and patients
• Administer medications and treatments as ordered, and monitor patient responses
• Document patient assessments, vital signs, and care provided accurately
• Collaborate with RNs, physicians, and the healthcare team to ensure quality care
• Assist patients with daily living activities and promote independence
• Educate patients and families on care plans and health management
• Maintain a safe, clean, and organized environment

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania
• Current BLS/CPR certification (American Heart Association or equivalent)
• Strong clinical skills and a passion for patient care
• Ability to work independently and as part of a collaborative team
• Excellent communication and documentation skills

Knowledge & Skills

• Medication administration and dosage calculation
• Patient assessment and vital sign monitoring
• Electronic health record (EHR) documentation
• Strong communication and interpersonal skills
• Ability to work collaboratively in a fast-paced environment
• Compassionate, detail-oriented approach to care
