Roles & Responsibilities

• Provide compassionate direct patient care and monitor vital signs
• Administer medications and treatments as ordered by physicians
• Collaborate with RNs and healthcare team members to develop care plans
• Document patient care accurately and in a timely manner
• Educate patients and families on health management and wellness
• Maintain a safe, clean, and supportive environment for residents

Qualifications

• A current and active LPN license in Pennsylvania
• Current BLS/CPR certification
• Experience in long-term care or skilled nursing is a big plus
• Strong critical thinking and communication skills
• Comfort working flexible per diem shifts including nights

Knowledge & Skills

• Medication administration and dosage calculation
• Patient assessment and vital signs monitoring
• Wound care and infection control practices
• Electronic health records (EHR) documentation
• Compassionate bedside manner and patient advocacy
• Team collaboration and clear communication
