Roles & Responsibilities

• Provide direct patient care and assist with activities of daily living
• Administer medications, treatments, and immunizations as ordered
• Monitor and document vital signs, symptoms, and changes in patient condition
• Collaborate with nurses and physicians to implement patient care plans
• Educate patients and families on health management and follow-up care
• Maintain accurate, timely patient records and ensure a safe environment

Qualifications

• Current and active LPN license in Pennsylvania
• BLS/CPR certification required
• Graduation from an accredited practical nursing program
• Ability to work the 3:00 PM - 11:30 PM shift as needed
• Strong clinical assessment and communication skills
• Compassionate, patient-centered approach to care

Knowledge & Skills

• Medication administration and dosage calculations
• Patient assessment, vital signs, and charting
• Wound care and infection control practices
• Electronic health records (EHR) systems
• Care plan implementation and patient advocacy
• Effective communication with patients, families, and the care team
