Roles & Responsibilities

• Provide direct, compassionate nursing care to residents in accordance with facility policies
• Administer medications and treatments safely and accurately
• Monitor, assess and document vital signs and changes in patient condition
• Perform wound care, dressing changes and basic procedures as indicated
• Collaborate with RNs and the interdisciplinary team to support individualized care plans
• Educate patients and families about care routines, medications and safety
• Maintain accurate, timely documentation and ensure regulatory compliance
• Support a safe environment by following infection control and safety protocols

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico
• Graduate of an accredited LPN/Vocational Nursing program
• Current Basic Life Support (BLS) certification or ability to obtain quickly
• Comfortable working overnight 12+ hour shifts (7:00 PM - 7:30 AM) on scheduled days
• Local-only requirement: must be able to commute to Albuquerque Hts HC
• Strong communication skills and ability to work collaboratively with a care team
• Previous experience in long-term care or skilled nursing preferred

Education

• Diploma or certificate from an accredited LPN/Vocational Nursing program

Knowledge & Skills

• Medication administration and basic clinical procedures
• Patient assessment, vital sign monitoring and early recognition of changes
• Wound care and infection control best practices
• Accurate clinical documentation and charting
• Teamwork, clear communication and empathy for patients and families
• Time management and the ability to prioritize in a busy setting
